Community Team for Adults with Learning Disabilities Bridgwater, Minehead, Sedgemoor and West Somerset
Care and support, Information, advice and guidanceDescription
We assess a persons requirements and provide statutory care and support as needed for adults over 18. Our aim is to promote a persons wellbeing, independence, choice and control, with the support of a transitions worker for under 18s.
We safeguard vulnerable adults who are experiencing abuse or neglect.
We can provide a range of specialist health care support including psychiatry, psychology, speech and language therapy, learning disability specialist nursing and occupational therapy to support people with a learning disability.
We support a person who may be caring for someone and is not being paid to do so.
We assist people through the provision of equipment, telecare and housing adaptations. Depending on a persons needs, we also arrange the provision of appropriate accommodation, for example, independent flats, supported living and residential care.
