Children with Disabilities social work teams
Care and supportDescription
Social workers support children whose disability means they have multiple and complex needs that significantly impacts on their daily life.
The social worker will meet with you and your child to complete a Children and Families Assessment to better understand your child’s and family’s needs. This will include, with your consent, talking to any professional already involved with your child. Following the assessment, should a need be identified, and your consent obtained, a Child in Need plan will be completed with you, which will detail agreed actions. Your child’s plan will be reviewed every 3 months.
Where concerns are raised about a child’s safety resulting in the need for more support, greater care and/or protection, social workers may need to carry out an investigation under Section 47 of the Children Act (1989) so that plans can be put in place to keep the child safe.
