Children with Disabilities Early Support Teams
Care and supportDescription
Family Intervention Workers will meet with you and your child to complete an early support record and plan to better understand your child’s and your family’s needs. Following this, they will carry out direct work with your family and offer support and guidance for a variety of needs. Your child’s plan will be reviewed every 6 months.
Family Intervention Workers will facilitate and attend Team around the Family meetings (TAF) to ensure all services are joined up to provide the right support for your child.
Many of our services can be accessed at early help level and do not require a full social work assessment. However, this will be offered if requested.
You can find more information about the team on the Somerset Council Children with Disabilities page.
