•    Full time (37 hours), all year round position
•    Hybrid working model with a balance of home, locality and office working 
•    Based in a locality but with the expectation that casework is needs based and can be across Somerset
•    25 days’ annual leave with less that 2 years continuous service, and 28 days with more than 5 years continuous service - plus bank holidays
•    Permanent contract

What will I be doing?

You’ll play a vital role in transforming lives in Somerset—making a real difference every single day! As a Transition Worker, you’ll be at the heart of supporting children and young people navigating neurodiversity pathways, including autism. Here's what your exciting day-to-day will look like:  

•    Creating tailored support plans: gathering detailed information from new referrals to create comprehensive support plans tailored to each child's needs.
•    Empowering families: working closely with parents and carers, offering one-on-one support and practical strategies that empowers parent carers and makes a lasting impact.  
•    Collaborating with professionals: attending and contributing to Team Around the Child (TAC) and Personal Education Plan (PEP) meetings, ensuring children and young people secure better outcomes.  
•    Bringing fun and support to life: organizing and assisting in engaging activities and clubs that foster growth and connection. 

What kind of experience or qualifications do I need?

We are committed to providing comprehensive training, guidance, and support to ensure you excel in this role. That said, having the following will really give you a head start:

•    A solid understanding of neurodiversity, especially autism, and its impact on children and families.  
•    Knowledge of education and transition processes to guide children through pivotal moments.  
•    Experience in crafting support plans, conducting home visits, and working with children in care and their families.  
•    A relevant qualification in education, social work, or a related field
•    Additional training in autism and communication strategies to demonstrate your dedication and expertise.
