Community Council for Somerset Village and Community Agents provide confidential, practical community-based solutions for customers. Can be contacted directly, or by referral through the Doctors surgery.

Community Council Somerset Community Agents can only be contacted by a referral from the doctors or Adult Social Care Services.

Long term support for vulnerable families and individuals including dedicated Link Worker and volunteer mentor addressing mental health, isolation, debt, benefits, family breakdown, anti social behaviour, school attendance and housing issues. They also have a resettlement worker working refugees, and a community outreach worker working closely with the street community and people who are vulnerably housed
